We would like to facilitate the evaluation and diagnostic process of your first or follow-up office visits. In addition, we aim to reduce your processing time. Here you have access to our registration, medical history, medical update, TMJ and Sleep Apnea forms. Please complete them prior to your appointment
Kindly download, complete and print the forms below, read the directions and questions carefully and provide as much detail as possible. For example, make sure that your medical history includes a complete list of all the medications you are taking, traumas, infections and surgeries you have had.
You may contact us with any questions and you may also fax your forms to us ahead of your visit. If you have any problems printing these files properly, we will be happy to mail or fax them to you.

We understand that you have a busy schedule, so when you’re preparing to visit the dentist please remember that your appointments can last 1-2 hours.
The day of your appointment BRING YOUR SMILE!!! But also remember to bring these with you to your appointment:
